Church Street Surgery is a Very Low Cost Access Practice. The following fees are charged to Church Street Surgery enrolled patients, for seeing our doctors for an ordinary consultation :
Children 13 years old and under = FREE
Youth aged 14 to 17 years old = $13.50
Adults 18 years and over = $19.50 for a 15 minute GP/Nurse Consult.
These charges relate to an ORDINARY CONSULTATION, additional charges sometimes apply.

If you are in an Emergency situation, call 111 and ask for an ambulance.

If you require medical assistance and Church Street Surgery is closed we have a few options for you:
- you can call the surgery number 073156307. This call will be diverted to a Registered Nurse and she will triage you advise what to do.
- Call the Opotiki medical hub on 07 3158500 they have a nurse present Mon - Fri 5pm to 10pm Sat - Sun 8am to 10pm with a doctor oncall via phone.
- Call Healthline free on 0800 611 116 for health advice and information 24 hours a day, 7 days a week.
- Present to Whakatane hospital A & E where a nurse will triage you.
